I'd like to rerserved the PCR test for two people at 14:00 on Monday, August 22nd. 

As I couldn't type in their name and phone number into your online reservation form, Please let me know how we can reserve the PCR test for the above two.

This PCR test is for 1. Korea within 1 day upon arrival (need to upload the data into Q-code App). and 2. entering into JP (72hrs before), and we need both data/ original certificate copy.

As Japan's requirements are strict, attached here their sample file.

Please let me know how I can reserve the PCR test on their behalf.   

Please also let me know if you can cover the attached regulations.
